The form below could be printed out, completed and brought in prior to your first session. Doing so will avoid taking time from your session.

Click the link below. Click on the link in the center of the page that says 'Chakra Test'. Complete the test and select 'Results' at the bottom of the test. Scroll down the 'Results' page and you'll see 'Send these results to your email address'.  Fill-in renee@restoration-you.com and click send. We will use these results as a part of your Chakra balancing session. The results should be completed and emailed to me prior to your energy healing session. You can also print the results and bring them in on the day of your session.
